1. A recent, colour, passport-size photo (i.e., approximately 40mm (Width) x 50 mm (Height) showing a full frontal face with clear facial features.

2. Copy of your identity documents, e.g. HKID Card, Passport, Mainland Resident Identity Card, Exit-Entry Permit for Travelling to and from Hong Kong and Macau, etc.
身份证明文件： 香港身份证、护照、内地居民身份证或往来港澳通行证副本。

3. Academic Qualifications: Graduation Certificate(s) and/or Degree Certificate(s).
学历证明：所有大学学历的毕业证书及学位证书副本。
 

a. For qualifications from Mainland China: You must also provide the Online Verification Report(s) issued by China Higher Education Student Information (CHSI). (Ensure the online verification codes remain valid for at least 6 months).
内地学历：须同时上传由中国高等教育学生信息网（学信网）发出的有关的所有验证报告（请确保上述三份报告在未来至少六个月内有效）。

i. China Higher Education Student's Academic Transcript;
《中国高等学校学生成绩验证报告》；
ii. China Higher Education Qualification Certificate;
《教育部学历证书 电子注册备案表》；
iii. China Higher Education Degree Certificate;
《中国高等教育学位在线验证报告》；
iv. If you are currently studying the programme, you should provide the following verification: Online Verification Report of Student Record.

b. For Mainland Chinese applicants with overseas qualifications: You must provide the Overseas Credential Evaluation Report issued by the Chinese Service Center for Scholarly Exchange (CSCSE) .
内地申请者持境外学历： 须上传由教育部留学服务中心出具的《国外学历学位认证书》。
c. Applicants with dual-degree must hold a graduation certificate/testimonial with name of the collaborating institutions.
持双学位课程申请者： 须持有注明合作院校名称之毕业证书或证明文件。
d. For applicants with qualifications obtained in Hong Kong or overseas institutions: Copies of the original Graduation Certificate(s) and/or Degree Certificate(s) must be provided.
持香港或海外院校学历申请者： 须提供毕业证书及/或学位证书副本。
 

4. Academic Transcripts: Copy of official Transcript(s) with a grading scheme from all tertiary institutions attended.
成绩单： 所有专上院校的正式成绩单副本（须附评分标准说明）。

5. Proof of Language Proficiency: Copy of score report of TOFEL/IELTS and other language tests (if applicable).
语言能力证明：托福或雅思或其他语言测试成绩单副本（如适用）。

6. Personal Statement and Curriculum Vitae (CV) .
个人陈述及个人履历。

7. Two referees’ contact details (academic and/or professional): *Please provide valid institutional contact information for your referees (e.g., official university or office email address and telephone number). Personal/non-institutional contact information is not preferred. Please ensure your referees can receive messages sent from Hong Kong. Failure to do so may delay or adversely affect your application.
两名推荐人信息（学术及/或专业）： 必须提供推荐人的有效官方联络方式（如大学或工作单位 电邮及电话）。不建议推荐人使用个人电邮收发本校讯息。请确保推荐人能正常接收来自香港的电邮，否则可能影响申请审核。)

8. Copy of a valid visa/entry permit (e.g., IANG, working permit or dependant visa) if currently held.
有效签证/入境许可如现时持有（例如：IANG 签证、工作签证或受养人签证）。

9. Copies of certificates of professional qualifications or proof of professional membership (if applicable).
专业资格证书/专业学会会员证明副本（如适用）。

10. Other supporting documents (if any).
